Conformation Registration Form

Please fill out the form below to enroll your child in preparation for their Confirmation. Please note that your child must have received their first Reconciliation (Confession) and their First Holy Communion in order to receive the Sacrament of Confirmation. If your child was not baptized or did not receive Reconciliation or First Holy Communion at Holy Trinity Parish, we will also need a copy of your child's baptismal certificate (with notations of other sacraments received), which can be obtained from the parish in which your child was baptized.
