First Holy Communion Registration

Please fill out the form below to enroll your child in preparation for their First Holy Communion. Please note that your child must have received their first Reconciliation (Confession) in order to recieve their First Holy Communion. If your child was not baptized at Holy Trinity Parish, we will also need a copy of your child's baptismal certificate, which can be obtained from the parish in which your child was baptized.
